Criteria
for Sheltered Care | |
1. | Resident
must be able to move independently within their room, to meals and activities. |
2. | Resident must be able to dress/undress self and maintain good grooming with minimal assistance. |
3. | Resident must be able to go to bathroom without staff assistance. |
4. | Resident must be continent of bowel and bladder. |
5. | Resident must be safe alone in their room without continual observation. Nursing care is on a custodial basis at irregular intervals. |
6. | Resident must live compatibly with other people in a group living environment. |
7. | Resident must be mentally alert and oriented, able to locate their room without assistance. |
8. | Resident must be able to maintain daily personal hygiene with minimal assistance or reminders. Assisted bathing provided twice a week. |
9. | Laundry provided for resident's clothing. Clothing must be marked and identified. |
10. | Resident must be able and cooperative about participating in some form of social activity within facitlity and/or community. |
11. | Resident must be able to feed self. |
12. | Resident shall not require mental treatment as defined in the "Illinois Mental Health Code." |
13. | Resident shall not hae a communicable, contagious or infectious disease. |
14. | Oxygen can be administered with the use of an oxygen concentrator, portable units if needed, or by a nurse in an emergency situation. No blood transfusions may done in the facility. |
15. | No person under the age of 60 will be admitted as a resident. |
16. | Any resident with a urostomy or colostomy must be physically and mentally capable of doing their own ostomy care. Each case will be evaluated professionally and individually. |
Discharge Policy
A resident will be discharged from Greenfield if their medical, physical or mental condition is such that the above criteria cannot be met, or for the resident's safety or for the safety of other residents.
The decision for discharge shall be made, with the resident's involvement, by the Administrator, Director of Nursing, resident's personal physician and family. Reasons for discharge shall be discussed with the resident and family, the Administrator, Director of Nursing and other pertinent staff. Twenty-one days written notice for other placement shall be given, except in emergency situations when discharge is mandated by resident's health needs.
